How is application and payment for AlliedT Short
Term MEDICAL PLUS made?
There are three ways to apply for the Allied Short Term Medical PLUS
1. Complete the online enrollment process.
2. Download then print, complete and mail the paper application and
3. Contact an Allied sales agent for assistance. Assistance
If you enroll online, the initial payment must be made by credit card (Visa or
MasterCard). If you enroll by mail, check or credit card may be used. The
Allied Short Term Medical PLUS Plan can be billed monthly, or you can prepay
for one to three months (the number of months you choose to be covered) and
receive a discount off the monthly rate. If you are not prepaying, subsequent
monthly payments can be made by credit card, auto withdrawal from your checking
account or billed to you.
Please note that when choosing the prepayment option that, after the 10 day free
Certificate of Insurance review, coverage can not be cancelled and no premium
paid under the prepay plan discount is refundable.
